When Abby becomes stricken with a severe rash and fever Connor and Hicks head for the nearby town of Bishop Falls in search of a physician where they are immediately drawn into a web of conflict, suspicion, and danger.

The town's only doctor is investigating the possible resurgence of a fever outbreak in the nearby mining encampment, and Reginald MacArthur, the prominent businessman who rules the town with an iron fist, orders them to leave. Faced with no other recourse, they take Abby to the miner's enclave on the mountain. The fearful miners are terrified of another epidemic.

Additionally, there have been several recent murders attributed to a huge, ghost-like creature of Indian legend that has been leaving a trail of horribly crushed bodies in its wake. While Abby battles for her life against a virulent pathogen, Connor and Hicks are trapped between a ruthless megalomaniac and his band of brutal gunmen, a group of resentful, near-hysterical miners, and a purported supernatural monster.
