Being involved in an automotive related accident is a stressful situation and we find that far too many individuals get caught not knowing what to do when this occurs. Therefore, we've created this log as a valuable reference and simple tool for collecting the necessary information for submitting an insurance claim. We suggest placing one of these logs in the glove box of each vehicle you own.

The pages of this log are designed in a "fill in the blank" format, with enough space for documenting up to 15 different accidents. The interior contains spaces for logging valuable information following an accident, such as:
- Date, Time & Location of Accident
- Other Driver's Contact Information
- Other Driver's Vehicle Information
- Other Driver's Insurance Information
- Description of Damage to Vehicles
- Passenger Information
- Witness Information
- Other valuable notes or drawings
